I am an in-network provider for Empire Blue Cross Blue Shield. If you do not have Empire, services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage for out-of-network providers carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- Do I have coverage for out-of-network providers?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Cash, check, Venmo, Paypal or Chase QuickPay are accepted for payment. Please note session fees and co-pays not payed at the time of the appointment are subject to a $5.00 surcharge.
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of a session (not the co-pay).
